Section 6.5 – medial mobilisation of mesosigmoid and left mesocolon
A region of peritoneal reflection occurs at the medial extremity of the mesosigmoid and left mesocolon
This must be divided through next, as part of detachment of the mesentery
The division begins distally, at the medial margin of the mesosigmoid near the midline
The division is continued proximally, at the medial margin of the left mesocolon
In this video clip the mesosigmoid has been fully detached, but not disconnected
The reflection at the medial margin of the left mesocolon is apparent, and must be divided
The reflection at the medial margin of the left mesocolon, the medial mesocolic reflection,
Is divided using sharp followed by blunt dissection
This completes detachment, but not disconnection, of the left mesocolon
Disconnection requires division of the inferior mesenteric arterial connection
